Apply for a Severe Mental Impairment Discount
You should have ready your Council Tax bill, so that you can fill in your council tax reference number on the online form.
You will also need to upload, when prompted by the form:
- a copy of the benefit entitlement letter from the Department for Work and Pensions; and
- the doctor's certificate which has been completed by a health professional.
If you don't have them yet, or you have not yet got the doctor's certificate completed, fill in the online form anyway and send them to us later.
To send them later you can:
- scan or photograph them and use our contact form to send them on; or
- post them to: Council Tax Service, PO Box 3, Manchester M15 BA.
After you've applied for the discount
You must continue to pay the amount on your Council Tax bill until you hear from us.
Tell us if your circumstances change as this may affect your discount. If you don't tell us you may have to pay a penalty.
