Do not use this form unless you have received a letter from the Manchester Revenues and Benefits Unit advising you to do so.

Within this form you will be asked about:

  • your income;
  • your rent;
  • the people who live with you as part of your household; and
  • and if anything in your circumstances has changed since you last gave us details.

You can upload evidence when you fill in the form if you are telling us about a change in circumstances to your existing claim.

Please have ready:

  • your benefit reference number, and
  • any scans or photos of documents you want to upload as evidence, on the device you are using.

Don’t delay filling in the form because you don’t have the evidence we need now. You can always add additional evidence at a later date
